Compounded peptides cost about $25 to $250 per vial at a pharmacy, according to thepeptidelist.com’s pricing guide. Clinic programs cost more, and one clinic’s injectable plans run $275 to $550 a month. Insurance covers only FDA-approved peptides, according to the same guide, so expect to pay for a compounded version yourself unless your plan confirms otherwise.
How much do compounded peptides cost per vial?
Pharmacy prices depend on the type of pharmacy, the peptide and the strength. The thepeptidelist.com pricing guide gives these ranges, and says its figures reflect market analysis at the time of publication. Check today’s price with the pharmacy before you commit.
| Type of pharmacy | Price range per vial | Semaglutide 5 mg |
|---|---|---|
| 503A compounding pharmacy (prescription required) | $25 to $140 | $80 to $140 |
| 503B outsourcing facility | $120 to $250 | $120 to $200 |
For comparison, the same guide lists brand-name Ozempic or Wegovy at $900 to $1,300 a month without insurance, and Mounjaro or Zepbound at $1,000 to $1,400. Those are FDA-approved brands, not compounded versions.
Peptides sold as ‘for research only’ are a different category, and they are not compounded. Our research labs peptides reviews cover their price and refund questions. Our research labs peptides alternatives guide compares those options on price and safety.
What does a clinic charge each month?
Clinic price lists often include the visit and the medication in one number. Skye Weight Loss Solutions lists the prices below on a page dated April 1, 2025, and says prices are subject to change at any time.
| Compounded medication | First four weeks | Later four-week levels |
|---|---|---|
| Semaglutide | $300, including the intake visit and a four-week supply | $275 to $400 |
| Tirzepatide | $325, including the intake visit and a four-week supply | $325 to $550 |
Starting on a dose above the first level adds a $60 new-patient fee, according to that page.
Alloy Weight Care lists compounded tirzepatide starting at $100 for a one-month supply, as of October 31, 2026. A $99 monthly fee covers unlimited messaging with its doctor, wellness check-ins, side-effect management and dose adjustments. The page offers $50 off your first month of Alloy Weight Care for new customers. It says the program is not available in AL, AR, CA, NV, LA, MS or DC, and describes it as designed for women in midlife.
Empower Pharmacy’s peptides page does not show a price; it asks you to request pricing. The same page says all its medications require a prescription from a licensed prescriber.
Does insurance pay for compounded peptides?
The thepeptidelist.com guide says insurance covers only FDA-approved peptides, and does so inconsistently. It adds that insurance covers monitoring for FDA-approved medications but rarely covers labs for compounded peptides.
Coverage depends on your plan, so call the number on your insurance card. Ask whether it covers the compounded version you are considering and whether it covers the labs.
The Obesity Medicine Association’s Obesity Medicine Association FAQ on compounded peptides lists high costs as one of three obstacles that limit patient access to peptide anti-obesity medications.
What costs come on top of the drug price?
| Extra cost | Range given |
|---|---|
| Initial telehealth consultation | $99 to $300 |
| Follow-up visits | $50 to $150 |
| Monthly monitoring | $25 to $100 |
| Initial labs | $200 to $400 |
| Standard shipping | $15 to $25 |
| Temperature-controlled shipping | $25 to $40 |
These ranges come from thepeptidelist.com’s guide, not from a clinic’s own list. Ask your provider which of them apply to you.
What is not known yet about compounded peptide prices?
Peptide rules and prices have changed before, and how future regulatory changes will affect compounded peptide prices or insurance coverage is not known yet. The thepeptidelist.com guide says FDA restrictions have changed pharmacy access and prices.
The prices above are a snapshot. Skye’s list is dated April 1, 2025, and the guide’s figures are estimates. For today’s number, use the quote from the pharmacy or clinic you are considering.
Before you order

- Ask whether follow-up visits, labs and dose changes cost extra.
- Call the number on your insurance card and ask whether it covers the compounded version you are considering.
